Overview

As we age, natural D-dimer levels increase, causing more 'false positive' results when using the standard 500 ng/mL cutoff for Pulmonary Embolism (PE) or Deep Vein Thrombosis (DVT). The age-adjusted cutoff increases the specificiy of the test without sacrificing sensitivity for patients over 50 years old.

If your lab uses DDU (D-dimer Units), the standard cutoff is usually 250 and adjustment is Age × 5.
D-dimer has high negative predictive value but low positive predictive value.
Never use D-dimer alone to rule in a clot; it only helps rule it out.
