Overview
The qSOFA (Quick SOFA) score is a simplified version of the Sequential Organ Failure Assessment (SOFA) score used to quickly identify bedside patients outside the ICU who are at higher risk of death or prolonged ICU stay when an infection is suspected.

Pro Tips
Altered mentation (GCS < 15) can be as simple as the patient not being fully alert or oriented.
If qSOFA is ≥ 2, the clinician should evaluate for organ dysfunction (SOFA score) and consider escalation of care.
Do not use qSOFA alone to rule out sepsis; clinical judgment remains paramount.
